  2. Download your New Patient Forms (forms to your right)
  3. Bring or email me any recent blood or labwork
  4. Read the FAQ's (see below) related to the type of appt you're scheduling

Nutrition Solutions and Food Craving Solution sessions are available via skype, iChat or phone or at my Los Gatos office.  Acupuncture is available in my Los Gatos, CA office.
